What is GERD?

Currently, GERD (Gastro-Esophageal Reflux Disorder) is one of the commonly observed conditions in youths due to various factors, such as faulty diet and lifestyle, excessive smoking or alcohol consumption, stress, etc. Gastroesophageal refers to a condition involving the stomach and esophagus. Reflux means backflow.

It is termed gastro-oesophageal because the condition involves the esophagus and stomach.

Scope and Duration of Treatment

Homeopathy may be considered as a first-line option in early-detected, mild to moderate cases of GERD, and as supportive care in chronic cases. Treatment duration varies by individual and depends on factors such as disease severity, duration, associated conditions, medications, and lifestyle. Mild to moderate cases generally respond faster than severe ones. Homeopathy focuses on symptom relief, healing, reducing the tendency for recurrence, and addressing both physical and psychological factors such as stress and anxiety. Many patients may notice improvement within six to eight weeks.

Common Medicines

Several homeopathic medicines are commonly prescribed for managing GERD, including Natrum Phosphorica, Iris Versicolor, Robinia, Nux Vomica, Phosphorus, Lycopodium, and Carbo Vegetabilis. The choice of medicine depends on the individual’s symptom pattern.

GERD Pathogenesis

Normally, food passes from the esophagus into the stomach through the lower esophageal sphincter, which remains closed to prevent backflow. When this sphincter becomes weak or relaxed, stomach acid and contents reflux into the esophagus, causing irritation and inflammation. This leads to symptoms such as heartburn, acidity, sour or bitter taste, and chest discomfort. Occasional reflux may improve with lifestyle or dietary changes, but persistent symptoms result in GERD. Factors like faulty diet, lifestyle habits, stress, hiatus hernia, obesity, and pregnancy can contribute to or worsen this condition.

GERD Prevalence

GERD is one of the most commonly diagnosed digestive disorders worldwide, affecting about 18–20% of the population. Females are more commonly affected than males, with a ratio of 2:1, and the risk increases with age. In India, around 10–30% of people are affected, with higher prevalence noted in the southern population. GERD also occurs in children and infants; physiological GERD affects nearly 70–80% of babies, often due to overfeeding, improper positioning during breastfeeding, or lying down immediately after feeding, and usually resolves as the child grows.

Causes

GERD develops due to multiple contributing factors, most commonly abnormal functioning or weakness of the lower esophageal sphincter. Conditions like hiatus hernia, delayed stomach emptying, stress, pregnancy, connective tissue disorders, obesity, and certain medical illnesses can aggravate reflux. Faulty diet, eating habits, and lifestyle factors such as smoking, alcohol use, lack of sleep, and inactivity also play an important role. Additionally, several medications may trigger or worsen reflux, eventually leading to GERD in susceptible individuals.

Symptoms

GERD commonly presents with heartburn, a burning sensation in the chest or behind the sternum, often worsening after meals, at night, or on lying down. Other symptoms include reflux or regurgitation of food or sour liquid, a sour or bitter taste in the mouth, sensation of a lump in the throat, nausea, chest pain, difficulty in swallowing, sore throat, chronic cough, hoarseness of voice, and bad breath. In children, symptoms may include regurgitation, vomiting, and persistent cough.

Complications

Poorly managed or untreated GERD can lead to serious complications. Continuous acid reflux may cause inflammation and scarring, resulting in narrowing of the esophagus and difficulty in swallowing. Ongoing acid exposure can also lead to erosion and ulceration of the esophageal lining, which may cause pain or bleeding. In some patients, long-standing reflux can alter the normal lining of the esophagus, leading to Barrett’s esophagus, a precancerous condition.

Diagnosis

The diagnosis of GERD is primarily based on clinical evaluation, including a detailed assessment of symptoms, medical history, and physical examination. To confirm the diagnosis and rule out other conditions, certain investigations may be advised. These include upper gastrointestinal endoscopy, biopsy of the esophageal mucosa, barium swallow X-ray, 24-hour pH monitoring, and esophageal manometry, which measures the rhythmic muscle contractions and function of the esophagus.

Supportive Treatment

Dietary, lifestyle, and habit changes play an important role in reducing GERD symptoms. Avoid spicy, pungent, fatty foods, excess onion, garlic, mint, coffee, chocolate, alcohol, and smoking. Eat small, regular meals, chew food well, avoid late-night eating, and do not lie down immediately after meals. Maintain proper posture, elevate the head of the bed, exercise regularly, manage stress, maintain a healthy weight, and ensure adequate sleep. Stay well hydrated, maintain a food diary, avoid tight clothing, include alkaline foods, and consider probiotics to support digestion and gut health.

Conventional Treatment

Conventional treatment for GERD commonly includes medications aimed at reducing stomach acid. Antacids are used to neutralize acid, though many can be habit-forming. H2-receptor blockers help decrease acid production for a longer duration, while proton pump inhibitors reduce acid secretion and inflammation. In cases where medications and lifestyle modifications do not provide sufficient relief, surgical intervention may be advised.
